Your self-pay office visit fee is credited toward a shockwave, PRP, or Lunula series started as a result of that visit. One credit per series. 
INSURANCE
These prices apply to self-pay services. Many of these treatments are not covered benefits under most insurance plans. If you would like us to verify your insurance benefits, we are happy to do so and will advise you whether coverage may be available and whether submitting a claim is appropriate based on your plan benefits. Coverage and payment are determined by your insurance carrier and cannot be guaranteed. 
SERIES & REFUNDS
Sessions in a package are yours until you use them; unused sessions in a started series aren't refunded. If a treatment turns out not to be indicated after evaluation, you pay for the evaluation only.
WHAT WE DON'T PROMISE
Results vary. Our physicians will tell you before you pay whether they think a treatment will work for your specific problem — including when the answer is no, or when a covered option is the better choice.
