The Myth (and why it lingers)

“I’ll just keep it covered with polish.”
“That nail’s been thick for years—no big deal.”
“If it doesn’t hurt, it’s just cosmetic.”

We hear versions of this every week in our South Tampa clinic. It’s understandable: fungus creeps in slowly, nails thicken bit by bit, and life is busy. But the truth is simple:

Truth: Onychomycosis (fungal toenails) is an infection of a living structure. Left alone, it often spreads, damages the nail unit, and sets up a constant cycle of detachment, debris, reinfection, and embarrassment. It’s not “just cosmetic”—it’s persistent biology.

What’s really happening under that nail

The nail plate is made by a factory (the matrix) and anchored to a living bed. Dermatophytes—or sometimes yeast/mold—invade the nail and its surrounding skin. The nail thickens, crumbly keratin builds up, and the plate can lift from the bed (onycholysis). Once detached, that air-filled space becomes a perfect moisture/food trap for the fungus to keep winning. That’s why “trimming it down” or painting it rarely changes the trajectory.


Bayshore’s Four-Part Protocol (what we do differently)

1) Start with proof: we culture the nail

Plenty of things look like fungus—psoriasis, eczema, trauma, even microbe-free dystrophy. Treating the wrong thing wastes time.
Our approach: We take a nail sample and send it for culture (and, when indicated, stain/PCR). This confirms whether it’s fungal and which organism it is. The result guides everything: laser cadence, topical plan, and whether we even need to discuss orals.

2) Accelerate healthy regrowth: EBM L1 nail growth support

You asked for your proprietary blend in this plan—done. In our experience, EBM L1 supports faster, healthier nail production and helps the new plate re-adhere to the nail bed sooner. Clinically, we see many patients roughly double visible growth rate versus doing nothing—so instead of crawling along at ~1–1.5 mm/month, we often see a noticeably quicker advance of clear nail.

4) Prevent recurrence as the nail grows out: Tolcylen®

Here’s the “gotcha” most people miss: even after the fungus is knocked down, the nail often remains partially detached for months while new plate grows in. That detached tunnel is a red-carpet invitation for re-infection.

We prescribe Tolcylen, a physician-dispensed antifungal and nail-conditioning topical, to:

  • Keep killing new invaders while the nail is growing out.

  • Condition the plate and peri-nail skin to reduce brittleness and micro-cracks.

  • Extend protection long enough for the healthy nail to outrun the problem.

In our protocol, Tolcylen is the recurrence insurance while Lunula and EBM L1 do the heavy lifting on eradication and faster re-attachment.

How long until my nail is clear?

Honest timeline: Toenails grow slowly. Even with better biology, we’re still working in millimeters per month. Most patients start seeing a healthier crescent at the base within 8–12 weeks. Full outgrowth for a big toe often takes 9–12+ months. That’s normal.

What we do is make those months count: reduce fungal burden early, speed healthy growth, and protect the nail while it re-attaches.


Is oral terbinafine still the “gold standard”?

Oral terbinafine remains a traditional benchmark for fungal eradication. But many patients either can’t take it or prefer to avoid systemic medications. Our Lunula-centered protocol gives you a non-systemic path with results we find clinically comparable in appropriately selected cases, especially when you commit to the full plan (laser + EBM L1 + Tolcylen + shoe/sock hygiene). We’ll discuss pros/cons and decide together based on your culture, health history, and goals.
